This is The ULTIMATE HDMI Switch from ViewHD.

VHD-PRO3X1i switch supports three HDMI Sources and one HDMI display with video resolution up to Ultra HD / 4K (HDMI v1.4 4096x2160@24fps or 3820x2160@30fps).

It provides advanced audio features to support a wide range of applications, including ARC, stereo analog audio output, TOSLINK optical audio (Dolby Digital / AC3, DTS 5.1CH) output.
