LEIS Driving Under the Influence (DUI) Field Notepad

The Driving Under the Influence Field notepad was developed to assist field personnel in taking adequate notes to generate a comprehensive Investigation back at the office while maintaining the confidence that all required information was obtained. Starting with the front cover and continuing through the notepad to the back cover, every detail has been thought of and scrutinized for its pertinence in the field. The cover is orange to distinguish the notepad as a DUI notepad The cover is laminated to resist the elements The lower corners are rounded to ease inserting into pocket The inside cover has printed Pre Field Sobriety Test (FST) questions in both english and spanish The front of the information pages include: First observations Arrest information Personal information Booking information Insurance and Vehicle information Pre-FST answers The back of the information pages include areas for nine different Field Sobriety Tests and passenger information: Horizontal Gaze Nystagmus One Leg Stand Walk and Turn Romberg Finger Count Hand Pat Finger to Nose ABCs Preliminary Alcohol Screening device Many have asked about other missing elements in this notepad. We have always reminded them that these notepads are intended for field notes. This notepad is typically used in conjunction with the Collision Investigation Field notepad.
